Changing your contact details

It's important we hold up to date contact information for you so we can continue to provide Safe & Effective care. 

To update details like your name, address, phone numbers or email address:

• complete an Online Consultation Admin request  
• phone or visit the surgery from 8am to 6:30pm

Please note, for name changes we will require sight of ID/Divorce or Marriage Certificates or Deedpoll documents. 

If you have moved to a new address, you will need to be in our catchment area to stay registered with our surgery.

To chnage your phone number or email address you can: 

• complete an Online Consultation Admin request  
• phone or visit the surgery from 8am to 6:30pm
• Email us on greensward.surgeryf81066@nhs.net 

You will need to show us proof that you have changed your name or date of birth. This could be a marriage or civil partnership certificate, deed poll or birth certificate.
